Resolution:

First, you will have to verify the issue. First ssh into the affected server. And run the following commands to get the logs:

omreport storage pdisk controller=0

omreport system alertlog | more

omconfig storage controller action=exportlog controller=0 (the output file will be in /var/log/   directory with the file name starting with "lsi_MMDD".log" ; where MM is the month and DD is the day)
